ATHLETIC CLEARANCE

CLEARANCE AND PHYSICAL INFORMATION

Need a Physical? Get a physical from your own doctor or you can call Sutter Health at (707) 427-4900 and make an appointment. Costs depend on your insurance. They are located at 770 Mason Street, in Vacaville. You can pick up a physical form in the Vaca Pena Middle School main office of the school or print out by clicking here for the Sports Physical Examination Form. Make sure you tell them that it is for a sports physical for Vaca Pena Middle School and take our school's physical forms with you.

ONLINE ATHLETIC CLEARANCE

  1. Visit www.AthleticClearance.com
  2. Watch the quick tutorial video
  3. Register. Parents register with valid email username and password. You will be asked to type in a code to verify you are human. If this step is skipped your account will not activate.
  4. Login
  5. Select "New Clearance" to start the process.
  6. Choose the School Year in which the student plans to participate. Example: Football in Sept 2023 would be the 2023-2024 School Year.

Choose Sport

  1. Complete all required fields for Student Information, Educational History, Medical History, and Signature Forms.
  2. UPLOAD PHYSICAL onto Athletic Clearance. Photos and PDF's are acceptable. Do not give your physical to your coach.
  3. Students need to complete a Student Social Media Course through NFHS. It's free! Then upload a picture of your certificate of completion on Athletic Clearance.
  4. Once you reach the Confirmation Message you have completed the process.
  5. All of this data will be electronically filed with your school's athletic department for review. When the student has been cleared for participation, an email notification will be sent.
 

INSURANCE

State Ed Code states that everyone must have medical insurance in order to participate in athletics. If you don't have medical insurance you must purchase school insurance in order to participate.

You can purchase the school insurance by doing one of these options.

  1. Click on: K-12 online enrollment.
    1. Pick the state of California and click look up.
    2. Scroll down to Vaca Pena Middle School
    3. Fill out and pay online
    4. Bring copy of insurance to Athletic Director, Katie Rondeau

OR

  1. Click on: Student Application for all other school districts, voluntary application.
    1. Print and complete the application
    2. Bring payment and application to Katie Rondeau, Athletic Director at Vaca Pena Middle School. 
    3. The Athletic Director will make copies and mail.
